Total Lifestyle Change, Inc. (TLC) is a nonprofit 501 (c)(3) organization that addresses health disparities that plague communities of color in underserved urban and rural areas.
We are seeking funding to begin our 2019 Education and Outreach Campaign. With a focus on families we will offer the following programs:
HOV TV Channel – Innovative programming that will be available via the internet and mobile phones. Topics will be Healthy Eating, Exercise, Diabetes Prevention and Management, Drug Abuse and Teen Violence.
Village Talk Radio Station focusing on mental health issues that are affecting our teenagers and young adults.
Diabetes Outreach and Screening through our Mobile Medical Van “STOP” Hypertension – we will track participant blood pressure using Bluetooth enable BP cuffs “Risky Business” – our Diabetes and Cancer Prevention Program Choices and Changes – our school-based fitness program.
Through these innovative programs and social media outlets, our goal is to reach 5 million families throughout the United States and Abroad.
Your donation will be used to provide health screening, educational workshops, and prevention programs at churches, schools, local businesses, community centers and local and statewide events throughout the United States.
For a donation of $5000, you will receive a HEALTH SCREENING and EDUCATIONAL WORKSHOP at the venue of your CHOICE.
